To SUBSCRIBERS, CLAIMANTS, AND CREDITORS OF PREFERRED HEALTH, INC.<\/p>\n\n\n\n
The Court of First Instance, Superior Court of San Juan, in Civil Case No. KAC09-0926 (904), has initiated a Liquidation procedure against the healthcare organization Preferred Health, Inc. (Preferred). To this effect, by Order issued on July 30, 2009, the Court appointed the Commissioner of Insurance of Puerto Rico as the Liquidator Administrator of Preferred.<\/p>\n\n\n\n
By virtue of this Order, any contract issued by Preferred that was in force as of the date of the Liquidation Order will be canceled. Any affected public employee due to this cancellation will have the right to select a new health plan from July 30th to August 31st, 2009.<\/p>\n\n\n\n
Subscribers, claimants, and\/or creditors of Preferred are also informed that they must submit their claims using a Claim Form, which will be available at Preferred’s premises, Central Office in San Juan, or at www.ocs.gobierno.pr.<\/p>\n\n\n\n
Any unpaid claims must be submitted using the Claim Forms, regardless of whether they were submitted before the date of the Liquidation Order.<\/p>\n\n\n\n
The completed Claim Form must be sent to the Liquidation Office, Urb. Caribe, 1550 Ave. Ponce de Le\u00f3n, Suite 2, San Juan, PR 00926-2791, no later than January 30, 2010. Failure to make a claim or provide the necessary evidence within the specified period will be deemed a waiver of the right to claim.<\/p>\n\n\n\n
