The Court of First Instance, Superior Court of San Juan, in Civil Case No. KAC2009-1424 has initiated a Liquidation procedure against the healthcare organization PROSALUD HMO, Corp. To this effect, by Order issued on December 21, 2009, the Court appointed the Commissioner of Insurance of Puerto Rico as the Liquidator of PROSALUD.<\/p>\n\n\n\n
By virtue of this Order, any contract issued by PROSALUD that was in force as of the date of the Liquidation Order will be canceled. However, subscribers to this plan are informed that the Office of the Commissioner of Insurance has authorized the transfer of PROSALUD subscribers to other healthcare plans with the purpose of not interrupting the healthcare services previously offered by this organization.<\/p>\n\n\n\n
Subscribers, providers, claimants, and\/or creditors of PROSALUD are also informed that they must submit their claims using a Claim Form, which will be available at the Office of the Commissioner of Insurance or at PROSALUD’s offices.<\/p>\n\n\n\n
Any unpaid claims must be resubmitted using the Claim Forms, regardless of whether they were submitted before the date of the Liquidation Order.<\/p>\n\n\n\n
The completed Claim Form must be sent to the Liquidation Office, Eleanor Roosevelt Avenue, 122 Interior, San Juan, P.R. 00918-3305, no later than February 28, 2010. Failure to make a claim or provide the necessary evidence within the specified period will be deemed a waiver of the right to claim.<\/p>\n\n\n\n
