The Court of First Instance, Superior Court of San Juan, in Civil Case No. KAC09-1569(905), has initiated a Liquidation procedure against the healthcare organization SDM Healthcare Management, Inc. (SDM). To this effect, by Order issued on December 17, 2009, the Court appointed the Commissioner of Insurance of Puerto Rico as the Liquidator of SDM.
By virtue of this Order, any contract issued by SDM that was in force as of the date of the Liquidation Order will be canceled.
Providers, claimants, and/or creditors of SDM are also informed that they must submit their claims using a Claim Form, which will be available at the Office of the Commissioner of Insurance or at SDM’s premises.
Any unpaid claims must be submitted using the Claim Forms, regardless of whether they were submitted before the date of the Liquidation Order.
The completed Claim Form must be sent to the Liquidation Office, PO Box 363031, San Juan, P.R. 00936-3031, no later than February 28, 2010. Failure to make a claim or provide the necessary evidence within the specified period will be deemed a waiver of the right to claim.
